Öncelikle emin olun çalıştığınız birimdeki tüm işleri ve kullanılan tüm evrakları 30 iş gününde öğrenemezsiniz. Örneğin; sizi stajda hasta kayıta verdiler ve göreviniz sadece hasta yönlendirmek. Peki kayıt yapan sekretere nelere dikkat ettiğini hiç sordunuz mu? Ücretli hasta gelirse ne yapmalı, mülteci gelirse ne yapılır, yanlış kayıtlar nasıl silinir vs. vs.
Hasta kabul çok karışık geldiyse başka bir birim olsun; cildiye polikliniğindesiniz. Riskli / özel rapor gerektiren ilaçlar var mı, varsa raporu nasıl çıkar, muayene dışındaki işlemler nerede yapılır, biyopsilerin girişi nasıldır, konsültasyona gelen hastanın kaydı nasıl? Gördünüz mü size bir çırpıda en az on günlük soru yazdım 🙂 Her güne bir tanesini öğrenseniz deftere aynı şeyleri yazmak zorunda kalmazsınız.
Röntgendeyim, sadece hasta çağırıyorum mu diyorsunuz? Hiç mi boş vaktiniz yok? Acile gidip kayıt nasıl yapılıyor izleyin. Gelen hastaların ve çalışanların tavırlarını gözlemleyin. Röntgenini çektiğiniz kemiğin Latince ismini not alın. İstedikten sonra emin olun çok şey öğrenebilirsiniz.
Yine staj defteri yazmayla ilgili sık sorulan bir soru: “Resmi bir üslup mu, samimi bir üslup mu?” Abartmamak kaydıyla size kalmış. Doğru ve anlaşılır bir Türkçe’yle yazmanız şartıyla üslup çok da önemli değil benim için. Tabi önemli değil derken “Ya hocam, neyse işte, aynen” gibi dost muhabbetlerinde kullandığınız kelimeleri de yazmayın 🙂
Gelelim defterdeki imza – onay kısımlarına:
1- Defterde “Mühür” yazan tüm yerlere staj yaptığınız kurumun yuvarlak mührünü bas(tır)acaksınız. Bu mühür Müdür ya da Başhekimde olur.
2- Defterin içinde çizgili bir sayfa var, o sayfaya staj yaptığınız her gün için kısa bir özet yazıp yanında çalıştığınız kişiye paraf / imza attıracaksınız. sayfanın en altına da kaşesini basıp imza atmasını rica edeceksiniz.
3- Staja gelmediğiniz günleri çizgili sayfada belirtmelisiniz. 3 iş gününden fazla devamsızlık yapanlar stajdan kalacak ve seneye tekrar staj yapmak zorunda olacaktır.
4- Rapor aldığınız günleri telafi etmek zorunda değilsiniz. ancak bu durumu birim amirinize kafa tutarak değil, güzelce konuşarak çözmelisiniz. Sonuçta bu kurumlar size mezun olduktan sonra da lazım. 😉
5- Okul açıldığında bana staj defterinizi eksiksiz olarak getirmeniz gerekmektedir.
6- Staj yaptığınız kurumda günlük olarak işe geldiğinize dair imzalatılan bir form olacaktır. Staj bitiminde o formun bir fotokopisini almalısınız. Staj defteriyle birlikte o formun fotokopisini de getireceksiniz.
7- Staj sözleşmesini teslim etmeyen arkadaşlar, staj defteriyle birlikte 3 sayfalık sözleşmeyi de getirmek zorundalar.
8- “Ziyaretçi Öğretim Elemanı” yazan sayfayı, ziyaretinize gelen hocanız olursa o dolduracak.
9- “İşin başlangıcı – Bitişi” kısımlarında sadece o günün tarihini başlangıç kısmına yazmanız yeterli. Bitişi ve işin yapılma süresi kısımları boş kalabilir.

Today, I went to the beachfront with my kids. I found a sea shell and gave it to my 4 year old daughter and said “You can hear the ocean if you put this to your ear.” She placed the shell to her ear and screamed. There was a hermit crab inside and it pinched her ear. She never wants to go back! LoL I know this is totally off topic but I had to tell someone!
